Crestwood Middle News
This is our last newsletter for the 2016-2017 school year! Thank you for Investing in Success with us! We are so pleased with our students! It was wonderful having so many families attend our End of Year Award Ceremonies! We were able to celebrate the great work of our students! It was also wonderful to see so many families attend our PTA meeting. Please plan to be an active part of the CMS PTA next year. We look forward to the 2017-2018 school year! We will continue to Invest in Success! Celebrate! Motivate! Support! Have a safe, relaxing, and rewarding summer! Don't forget to encourage your children to read and practice their math throughout the summer--We want them to continue to be successful!
Plan to attend our Open House and Back to School Night, when you will be able to pick up your child's Chromebook. You received a packet of information about CMS being a 1:1 school, and lots of information was included in the packet, including the technology fee of $60. Please keep the packet, and return the signed paperwork at our Open House on August 23, 2017, from 7:30 AM-4:30 PM or at Back to School Night on August 30, 2017, from 6:00 PM-7:30 PM. We are pleased to be able to provide this tool for our students!
Student schedules will be available via the Home Access Center (HAC) starting August 22. If your child would like to pick up his/her schedule, they can do so on August 23!
CMS has a partnership with the Maryland Safe and Supportive Schools (MDS3) program, and we work with this Johns Hopkins initiative to support our PBIS program. We are currently having students complete a survey at school regarding culture and climate, and we are also asking parents/guardians to complete the survey. The survey will require approximately 10 minutes for parents/guardians to complete. Parents/Guardians, please complete the survey on any computer and at any time that is convenient for you. The survey will be open until June 15. The parent/guardian survey is anonymous and voluntary. To access the survey for parents/guardians, please us the following link:

THANK YOU!!
Redistricting maps are posted in the front entrance at CMS. The maps show the new boundary lines for CMS. The BOE is scheduled to make their decision in June 2017.
The Crestwood PTA would like to thank everyone, especially our CAVS families, who stopped by our Summer Kick-Off last night. We were lucky to have great weather, and hopefully this holds out for the rest of the summer.
We'd also like to thank every family for your support throughout the year. We're already thinking about how we can help Crestwood Invest in Success next year. To help with that, we'd really like your feedback. Please take our quick survey at: https://www.surveymonkey.com/r/226ZDHH All responses will be kept anonymous, but we hope to use your feedback to keep growing the PTA. Enjoy the summer, and thanks again!
CMS is proud to announce that we received the Gold Award for our PBIS program. We are so proud of staff, students, families, and the community who have Invested in Success! We will be receiving the banner at a ceremony in August! Thank you for your support!

Yearbooks are currently for sale for $35 (cash or check made payable to CMS) on a first come, first served basis. Stop by Mrs. Grossnickle's room to purchase a book.
For the last two years, CMS has been fortunate to have a positive connection with the George L. Shields Foundation. Bob Brenengen, a board member, has advocated for two straight years to the rest of the board for our school to receive $10,000 each year to replenish and re-shape our fitness rooms. With that money, we have acquired 14 new rowers and 6 new bikes! Thank you, George L. Shields Foundation, for Investing in Success!!
ATTENTION PARENTS/GUARDIANS
School is scheduled to close June 9, 2017
Students are not permitted to take their medications home at the end of the school year (FCPS Regulation 400-23).
An adult must pick up all medications currently in the health room by the end of the school day (3:15 pm) on June 9.
Medication remaining in the health room will be destroyed on June 9th by 3:30 pm.
PREPARING FOR THE 2017-2018 SCHOOL YEAR
Medication/Treatment Orders from your Health Care Provider
The appropriate medication/treatment authorization form(s) must be renewed at the beginning of each school year. Forms are available online at www.fcps.org or at your child's school.
Please be aware of the expiration date on the pharmacy label when bringing the medication to the health room with the new authorization form. School health staff will determine medication expiration by the date printed on the pharmacy label. Note EpiPens, Diastat, Glucagon and inhalers have an expiration date printed on the container in addition to the pharmacy label. Therefore the container expiration date must be equal to or exceed the expiration date on the pharmacy label. Please make sure the expiration date on the individual medication container exceeds the expiration date on the label before leaving the pharmacy.
To ensure your child's medication needs can be met at the start of the school year, submit all completed forms and medications to the health room during the week of August 28, 2017 between 9:00 a.m. and 3:00 p.m. School health staff will review these items for completeness and discuss any issues at this time. Please do not send your child to school with medications. It is the responsibility of the parent/guardian to arrange for the safe delivery of medication to the school health room; and the parent/guardian must bring medications and forms directly to School Health Staff per (FCPS reg 400-23).
Thank you for your cooperation.
DON'T DELAY!! MAKE THE APPOINTMENT TODAY!
We do not want to exclude your child from school in September
State vaccination requirements for students entering 7th grade for school year 2017/18 have been revised. Your 7th grade student must have the following:
*Tdap is not the same as the other Tetanus, Dipheria and Pertussis vaccines called DPT or DTap. Even if your child has received DPT or DTaP in the past they will still need a single dose of Tdap.*
*We do not have documentation of your child receiving Tdap and MCV vaccination.*
Make your child's appointment now and avoid the rush. Please bring the vaccination documentation to school as soon as possible.
If you have any questions about the new vaccination requirements, please contact your child's health care provider or the school health room. Thank you.
Research shows that regular school attendance makes a difference in students graduating from high school. Our teachers prepare quality lessons each day for our students. It is critically important that students come to school on time ready to learn. Our goal is that each day for each grade level our attendance is above 96%. We need your help! Please encourage your child to attend school each day. If you need help, contact your child's school counselor. We are ready to help and support when needed. Attendance for May 29-June 2:
